Nonmember asking member friend to do ordinance work for deceased wife.

Joan Fackrell said: A nonmember friend who converted christian (He is Jewish.) has shown interest in the church. We have given him his own Book of Mormon to read. He lives in Missouri and we live in Utah. He unexpectedly asked if we would take his deceased wife's name to the temple and do her ordinance work. We feel this could be a good step in the right direction for him, in making the step himself in being baptized. We read that their is a form that gives my wife permission to do the temple work for our nonmember friends deceased wife. Is this true and where to we find this?
