Permission Needed Form
I am very frustrated with this form. It does not have a box on the spouse for a "unable to locate" living spouse for permission. I have the son's permission for my niece to be sealed to her mother and step-father, BUT I can't submit the form after trying FIVE times, unless I put all the information on the living spouse, which is impossible.

Im guessing living spouse permission is probably required - and not optional.

I am tried to have to permission for Sedun "O" (LX6G-6p4), But I can not have to found the permission form.How make reservation for temple work. He dead very young age, recent my wife live and she remember
Other question:
I found problem as follow,
Miss BAE (KN46-9LG), Miss KIM (KN46-Y5H), These tow name is not permission for reservation, reason is 'no first name' But other peoples did have this problems
Exam: Miss BAK (KN46-H12) This name was indicate possible to Temple work.
If no first name can not work, How do this work to do for reservation?
Please let me know how to work better.

For your second question: You cannot use a title and a last name. You cannot use Miss as a first name. (See: https://www.familysearch.org/en/help/helpcenter/article/how-do-i-format-names-to-avoid-the-needs-more-information-error-when-reserving-ordinances )
To be able to reserve the two daughters, remove Miss from their names and have just their last names entered. I have no idea why your third example allows the reservation despite have Miss in the title.
