Should apostrophes be included in surnames such as O'Hara, or a Given Name such as Mich'l?

I have been indexing such names with the apostrophe included to exactly duplicate what the record shows. I am reviewing a batch where O'Hara is indexed as OHara. I searched on this "Ask a Question" website and saw that this question has been answered but was restricted from seeing the answer. I've read the indexing instructions but am still not clear. I know that the "." is not needed when indexing a shortened name such as "Thos." I want to double check the latest direction concerning apostrophes.

I haven't found anything in the knowledge database which specifically addresses indexing apostrophes. But when we index a given name or a surname, we always type what we see.
