Is the Registar Date the same as the State Filing Date?

Hi, Claire! The State Filing Number is usually at the top right side of the page as you have typed for Image 1. The Registrar's Number is usually on the very bottom right or sometimes bottom left of the page and labeled "Date Received by Local Registrar". So for Image 1 it would be 12-4-1974.
Also noticed on line 12 of the form, there is a social security number. So in the entry for Social Security number, that should be marked as YES.
Also, for Residence and Birth Country, those should be marked <Blank>. The other day I indexed a death cert. which showed the man was born in Norway but was a US citizen. So Birth country and Citizen (line 9) can be different and should not be used interchangeably. Don't forget to check out the entry field helps. To do this, click on a field like Registrar Month and then click on the purple circle with the white question mark.

Thanks, Lisa. I'm relatively new to indexing, and still have issues sometimes. :)
I do love records that are typed out. They're much, much, easier than trying to decipher decades old handwriting.

You should join the Indexing Chat Group. There are many people with a lot of experience and who are wonderful to answer questions. Click on the Groups in the left column and then click on Join.

Don't forget to change the fields for Image 1 for Social Security Number to YES and the Registrar's date to be 12-4-1974. To make the change, just click on Image 1 and click on the 2 fields you want to change.
