Is the record I'm indexing a "Charge-Out" or "Certificate" form type?

    It is a certificate.

    On Image 1, it has been amended, so for the given name of the baby it should be Eva Beckie .

    Also, for Name of Hospital, follow the field help (purple circle with white ?) It says:

     If a hospital or institution was not included, but an address was, index this field as Home Birth.

    You might consider joining the Indexing Group.

    Look at the left column on this page. Click on Groups. Then scroll down the page to the box that says Indexing Chat. Click on it. Next, find and click the JOIN button. There is a lot of good information and the people are wonderful to help with indexing questions. Hope to see you there!
