Is it possible that batches have been mismatched with an image after indexing?

Have you seen any batches that are not connected to the right image after being indexed? I am reviewing a batch and the indexer has filled out all of the fields, but they don't match the image at all. (This batch is in Spanish)

Have you noticed this occurring on any other batches, or just this one? I would send the batch back for reindexing and wipe out all the data. But, I would also use the new form "Reasons to Reindex Batch" from Labs to alert FamilySearch. You can pull up that form with the batch open by clicking on the Help tab, click on Labs and follow the instructions to access the form. Since the indexer used the same information on both entries on two images, I suspect it isn't a problem with the batches being connected to the images after the indexing.
