In the field "Resident State" and "Resident county": Most leave that field blank or some put in an address but do not include the county or state, just a street address. If they do not specify the state or county where they are residents do I assume they are residents of the state where they died?

    The help fields on those fields say not to assume the place based on other information on the form. So even though they died in Wisconsin, we should not assume that was their residence.

    Those fields are not for the address. It asks for county or country, city, state.
