Amended birth cirtificate

As I started indexing Wisconson birth certificates, the first one was amended. The birth date was 1911 and the amendment made significant name changes for the birth child and the father's last name. The note with the certificate said the changes were made by a brother. What do I index?

    Welcome to the Community! You will indicate that it is amended. The general instructions tell us that if something is crossed out and something new is written, index the new information. If it is crossed out and nothing new is written, index what was crossed out if you can read it. I hope this answers your question.
