Should I include the father's first and last name in the 'Father's Surname' category?

I am indexing Wisconsin Death Records and I'm wondering if I should put the father's first and last name in the 'father's surname' category. I'm kinda new to this, but I've just been doing last name. Is this wrong?? I've already submitted around 4 batches this way and I'm worried I did it wrong, please help!

    You should on only type the surname as per field instructions (purple circle with white question mark)

    Type the surname as it was written. You are doing it correctly.
