How to index a certificate of examination

I have a record that has what I think is considered a certificate of examination but I don't know how to index it. Any help would be appreciated.

    It's actually a Witness Deposition.

    Image 1:

    GN: Walter
    SN: Charlesworth
    Record Day: 25
    Record Month: July
    Record Year: 1940

    Image 2 is a duplicate of Image 1 as it has the exact same information as Image 1.

    Please show a batch number so someone can help you. The number is at the end of the batch such as xxxx-xxx.
