What do I put in for record place?

It reads: "To the Honorable the District Court of the United States, holden at Province, within and for the District of Rhode Island"
I don't see a record number on any page. Do I just put "blank"?
Do I put the witness name for the given name on an additional entry, not the name of the person who is being naturalized?
Also, I put down accidentally that there was a second entry for image one. I left it blank and now when I try to submit the batch, it is showing 9 issues remaining, all of which are on that second entry. What do I do about that?

This is a Declaration of Intention, which does not have an example, so err on the side of caution and index it. It as Providence, Rhode Island, United States.
If there is no record number, mark it blank by either Ctrl+B or the fourth icon from the left on the toolbar.
Witnesses are not indexed on any naturalization document. Only the person seeking Naturalization is indexed.
Most projects today come with a preset number of entries. To delete the blank, unused entries, do the following:
Click the trash can icon on your toolbar.
This dialog box will appear. Choose "All blank entries" and click Delete. Once the blank entry(ies) are deleted, you will be ready to submit the batch.

I sent this in to be looked at. We'll see what can be done about the Project Instructions. In the meantime, erutherford gave you a great answer for how to take care of this batch.

There is an example for this project now for Declarations of Intention. It states that if the information for a field is missing to mark it as blank and the example shows what should be included on the image. Thanks @erutherford for your great answer!
