Johan,
Records read an index of her birth record. As you can see, she was born in Zamieście ( part of Tarnow). Her first name was Ludovica. Unfortunately, FS reads birth records from Zamieście only till 1879.

Ludovica Dzierwa f. Joannis was born in Zamieście on April 8-th 1891. Date you have provided is correct.

FamilySearch reads births from Zamieście only till 1879 :

This is why you need to contact Diocese Archive in Tarnow if you want to get a scan of her birth record.
