Today, a Chicago area death/burial has miraculously been transported to the West Midlands of England. And, it made many detours along the way.

Sorry I did not reply, but this collection was reported to engineers on October 4th. Here is the description that went with it in case you have anything to add:
Illinois, Archdiocese of Chicago, Cemetery Records, 1864-1989
Death place auto-changed on hundreds of thousands with event place originally indexed as places in the area of Chicago, Illinois. System is using the name of cemetery or burial place to create new incorrect event place.
Examples: 75,000 for Resurrection Cemetery standardized as "Resurrection, Kapisha, Chingola, Chingola, Copperbelt, Zambia.
49,000 for All Saints Cemetery standardized as parishes named All Saints in Bedfordshire, Staffordshire, Norfolk, Yorkshire, Leicestershire, and West Midlands, England.

In some cases, All Saints Cemetery also changed to Zambia -

Thank you for additional info. To answer your question about reporting errors, I will just say that (in general) they are being sent to my team before going to the engineers. We are trying to avoid sending each separate error found by a user as that creates an overload for the engineers. It's much better to try to identify the problem and how many records are involved so that the priority can be assessed and the right team assigned. That's why we need a community of experienced researchers giving us feedback.
