Where can I find "Welsh American Index" by Gethin Matthews?

The book or article in the question line is listed on the "Find a Welsh Emigrant or Immigrant from Wales Record Selection Guide" page on the FamilySearch wiki. It doesn't seem to exist, despite several searches here, WorldCat, and on Google.

    It's clearly disappeared from its original location on the web, but there's a copy on the Wayback Machine here:

    You'll need to wait (and ignore various loading and redirecting messages) for the individual pages to come up, but they do seem to in the end.
