What should I index from this immigrant record
The only part of this record I have been comfortable with indexing is the name, as it is the only thing listed that is either relevant or certain.
For instance, it lists a country, but at the top of the box it says 'country of birth "or" allegiance.
It has a date on it that is listed as the 'date of naturalization' which I have seen put into the record date, and the birth date, but it is neither of those things. Should I put it into the record day section?
Aside from that, the information either does not have a location to put it, or the information that is supposed to be there is 'not shown on F/O'.

Country of birth or Allegiance is open-ended, meaning it could be either or both. From the Field Help (purple question mark above the Birthplace Field):
When indexing the birthplace, please remember that the terms "late of," "native of," or "foreign" do not necessarily indicate the place of birth. Terms such as "nativity" or "country of nativity" do refer to the birthplace.
Easy example is people born in Canada have Allegiance to England because they were, and are, a part of the Commonwealth. Unless it specifically states "Canada" on the image, do not index the birthplace when you see "country of birth or allegiance."
Date of Naturalization is just that. Date of Naturalization. It will be put in the Record Date on the Oath of Allegiance, which was the day the Petitioner became a naturalized citizen.
You will not index everything on the document. If we did that, we'd still be on WWII draft cards (that ended a few years back). Index what is required and nothing else. The researcher will be able to see things like occupation and vessel name when s/he views the document. If a field is not there, like the spouse's surname, mark it <BLANK> by either clicking the fourth button from the left on the toolbar or Ctrl + B.
