I was indexing a naturalization record for a male from Poland. The signing official annotated on the record that the petitioner was changing his name to Robert Cummings

If you are indexing the Ohio project, you will follow the How To Index A Name Change Record in the Project Instructions under What To Index. You will index the date of the document (usually found in the upper right-hand corner, but can be located elsewhere in the document) and the name the gentleman is changing to; in the case it will be Robert Cummings.

If you are indexing any other Naturalization project, you will disregard the name change record.
