how to remove "familysearch template app"

Patron click on a pop up in the address bar, and it added "familysearch template app" to Chrome's address bar next to the star.

Now every time anyone logs into FamilySearch.org the square icon with an arrow in it appears in the address bar next to the star (for saving bookmark).

If you click on the square with the arrow in it, the window says "open with" and has the option "familysearch template app". If you click on "open" it opens another "window" and the patron has difficulty logging off.

How can it be deleted?
