Concerning: US, New York—Eastern District—Naturalization Records, 1899–1955 [Part F][MQNP-RN3]. When a Declaration of Intention and an Oath of Allegiance are on the same page of a two paged image. The Declaration of Intention and the Oath of Allegiance include the same given name and surname but the record dates are different. Should I treat this as one record since it is for the same person and use the most recent record date? Or should I create separate records for the Declaration of Intention and the Oath of Allegiance?

You will index them as separate entries.

Image 1, entry 1: These are two separate entries. The OOA has a different date.

Image 1, entry 2: On the second page, Affidavit of Witness, we do not index. Under that, Leonardo's info is in the "----- being duly sworn and deposes and says" paragraph.

Image 2, entry 1: An OOA is indexed with the First and Surnames and the record date.

Image 2, entry 2: This is a Certificate of Admission, which is not indexed.

Does this apply (indexing two separate entries) if the dates of the DOI and the OOA

    are the same? [MQJ9-2NW] batch/2c2645fa-06df-486f-809d-7634a2e443ef

This falls under indexing the DEC and PET as one document. It falls under this instruction:

When combining information from multiple non-overlapping documents on the same image for the same individual, use the record number and record date of the Petition for Naturalization.

You will combine the DEC and PET (on page 2) and enter the information from both documents (ie, any name variants) and use the date of the Petition as the Record Date. The OOA is a different, stand-alone document that is indexed as such

So, if I understand, image one will have one entry for the DEC & PET using

Sep 10, 1906 as the date. Then entry two (image one) will be the OOA using

Sept 12, 1906 as the date. Then entry three (image one) will be Shaike Schubak

and all of his pertinent info?

Thx for your help. I may have been doing these incorrectly and want to do them right!!

Yes. These are older records, and are out of sequence (compared to what is usually done), but a good way to remember is that the DEC was the first step ("first papers") in the two part naturalization process. The PET and the OOA were the second step ("second papers").
