US, California—Naturalization Records, 1903–1991 [Part A]

I am working on reviewing naturalization records and some of the given name areas list the given name and also include "or" For example given name: Giovanni Donatelli and lower in the document it says: I request that my name be changed to: George Donatelli

Should I list the given name as Giovanni or George?

Also, where do I find the record number? Is it the petition number and the certificate number on the Oath of Allegiance?

    You will index each unique name, separating them by the word Or. You will index it as:

    GN: Giovanni Or George

    SN: Donatelli

    The record number is the Petition Number on the PET. The OOA, while having a certificate number, is not indexed with a number and will be marked <BLANK>.
