    SN: Shapiro Or Stevens Or Szapiro. Every variant of a name, given and surname, is to be be indexed.

    Sex: Female. This is located in #6.

    Birthplace: Chudnow, Russia. The entire birthplace is to be indexed.

    Image 2:

    SN: Shapiro Or Szapiro Or Stevens.

    Record Date: January 9, 1945. With PET's, the Record Date is taken from the Affidavit of Witness Date.

    Record Place: Brooklyn, New York, United States of America. Eastern District is not used in this project.

    Also where in the document I can find the affidavit of witness date? I'm having a hard time finding it.

    The Affidavit of Witnesses is probably on the next image, i.e., you'll need to use the reference images to view it (but those are not available when viewing a shared batch, so I think erutherford must've gone to the unindexed images to get it, and I'm not that motivated).

    The Affidavit of Witness is located on the PET. Up until 1940, the PET had the Affidavit of Witness on the same page as the PET. Post 1940, the Witness section is located on the backside of the document, above the Oath Of Allegiance.
