Several questions on US, Massachusetts—Naturalization Records, 1790–1946 [Part A]

I just started on the US, Massachusetts—Naturalization Records, 1790–1946 [Part A]. There are two images.

The first image does not say what kind of document it is, so how do I know if it should be indexed? I couldn't find any examples for this type of document.

The second page of the second image appears to be a Certificate of Admission. It also doesn't say what kind of document it is, but it's very similar to the example for How to Index an Oath of Allegiance and Certificate of Admission, Example 1. In the example, they are only indexing the Oath of Allegiance on the opposite page. Does that mean the two are being combined into one record? Or do I create a second record for this image even though it's the same person?

    Image 1 is a Declaration of Intention. This is example 3 in the Declaration of Intention examples.

    Image 2 is an Oath of Allegiance. The language that you need to look for with this is do solemnly swear and declare on an oath that I will support the Constitution of the United States. This is example 1 under How to Index an Oath of Allegiance and Certificate of Admission.

    These older records (at least pre-1906) do not state what documents they are and you will not create two records and please remember to post your batch code in your comment.
