US, Rhode Island Nat Records, part G

At first glance, Image 2 appears to be a duplicate of Image 1, however, Image 2 has a different affidavit of witnesses at the bottom. It appears to have been added to the original document. So for the record date do I put 21 October 1921 from the first image or 19 Dec. 1927 from Image 2 ? If I put the October date then the second image is marked NNED. Or do I index them each separately using the two different dates? Please advise.

    I would index both, using 12/19/27 as the record date on Image 2. Image 2's Affidavit contains a third witness, James Kearns and for whatever reason, his affidavit was taken in 1927.
