In image 1 there is an OOA and a name change for 2 different people. I made 2 entries for image 1. Is that correct?

Yes, but for the name change, the Record Place is Brooklyn, New York, United States of America.

Why do the PI's not include Record Place in the sample provided?

Yeah, you're right. The place is blank, but it should Brooklyn, New York, United States of America. Unless they deemed that it somehow was not filed/taken place there. Going by what I was taught in school (ha!), the sender is Brooklyn's US District Court Clerk and the place should be Brooklyn.

That's the first name change I've done so the PI's are my only frame of reference. I'll hold onto it to see if you hear back from @Maile L .
