How to index these records [Zimbabwe, Bulawayo—Church Records, 1894–2022 [MQZL-3FD]]

I have been working on these Zimbabwe church records and I keep getting records such as this one that have no column headings and I cannot find an example of it in the Project Instructions. Does anyone know how we should index these?

    From the project instructions: "Indexes are images with only a name and reference information, such as page or volume number, with no other information, such as dates, available. Indexes should be marked as No, No Extractable Data."

    While this page includes a date as well as some sort of reference number, it is clearly an index. Unfortunately, shared batches do not have reference images, and they'd be unlikely to go back far enough anyway, so I don't think we can figure out what it's an index of. This means that we cannot usefully index the page, and I believe it should be marked "no extractable data".
