How do you index an oath of allegiance that has more information than the example?

I've been working on reviewing "US, Maine—Naturalization Records, 1790–1945", and the oaths of allegiance in these batches always have something attached at the bottom with the person's name, age, etc. The example in the project instructions doesn't have that and thus only lists the Given Names, Surname, and recorded month, day, and year. So, my question is, when additional information is provided on an attached piece of paper, should that information be recorded when indexed? [MQZV-MJJ]

You only index, or in this case review, the top most image. From the Important Details popup box:

Index only the topmost indexable record, or the overlay, on the image. If the document behind the overlay is also a record that should be indexed in this project, do not index the information on the partially covered document.

Image 1 has been indexed correctly. It is a variation of a Naturalization Certificate.
