Find a Grave does exactly what you want. You search for and find a name which has a photo of the tombstone and (maybe) some information about the person including where they were buried or what the disposition of their remains is. It depends on people taking the time to take an image of the tombstone and people to create accounts and provide the information. Is your intent that this QR code would require this be done all over again? Or would it just bring up the Find a Grave web page?
