the petitioner is requesting a name change. Do I index that as an alias? Please check my entry fields for GN and SN.

    Yes, for this project you do. Different state may vary. Your entries look good to me.

    On other state forms they would indicate whether the name change was approved in writing by the signature line of the Oath of Allegiance. If approved, there would usually two signatures (i.e., the applicant's signature before and after the name change). There you would enter the alias using the OR when required.
