I am looking for a blank 1900's Death Certificate form (model) so I can transfer over information

I am looking for a blank MICHIGAN 1900's (1933 to be exact) Death Certificate form (model) so I can transfer over information as I figure out the handwriting. I know I have seen blank forms for draft card, census records, etc. somewhere however google is not being helpful with this item. Thanks in advance for any help/direction.

    Err - they're all different so you need to tell us your target administration.

    Also, by "1900s", do you mean the decade or the century?

    Sorry - thought I entered it. Fixed my post to say for Michigan, and for the year 1933 to be exact.

    I would think your best bet is just to create your own. It shouldn't take very long to put together a spreadsheet that would work.

    Or take one and throw it into an image processing program and erase to get the fields you need. It tool less than five minutes to get this far:

    Erasing the rest would take about half an hour, I would guess.
