How to treat changed names on records

I am indexing naturilization records that have a persons name and then it say "formerly" and gives another name (i.e. Mary Johnson formerly Caroline Denny). Which name(s) do I use? I think it will be all of them as it is written!

    From the Field Help (accessed when in a field and clicking on the purple circle with the question market in it):

    • Given Name: Include all aliases, nicknames, or variant names that were given for a person, separating them with the word Or.
    • Surname: Maiden names should be typed in this field before the current or former surnames. A maiden name was often indicated by the words "née" or "born as." A maiden name may also have been written in parentheses.

    Given name: Mary Or Caroline

    Surname: Johnson Or Denny
