What should I put as the record date for image 2? There is an affidavit of witnesses on

reference image +1 but it does not have Helen Perkal's name anywhere on it so how do I know
it is for her?

Her signature is on the Oath of Allegiance, which makes it her record. You will use 6/21/1957 as the record date. Note that you will not use 7/23/1957; that is the record date for the OOA (and for whomever gets that batch).
