What purpose does a charge out sheet serve? the only useful information is the date of birth, but no name or parents names.

    In general, some charge-out sheets are a record of an adoption or other information added by the state sometimes months after the child is born. These do not always show the birth date or place, parents' names or other information shown on a birth certificate due to privacy issues. Check the project instructions and examples for how to index/review charge-out forms. 😎
