In batch [MQQ2-K2F] in the Michigan Naturalizations project

in the second record there are two names listed. I read the project directions for indexing a Petition for Naturalization document, and it says do not create a separate entry for other names. If the second name on the document is an additional name for the person on the record, as it appears in this document it could be a case of putting both last names with "or" between them as it says in the general indexing guidelines.

Should the second name be disregarded or should the last names be listed with "or" between them?

You will index it as Lopatin Or Lee. The example How to Index a PET Name-Change Record under What To Index shows an example of how to index multiple surnames.
