Some confusion on which date to use. There are 4 examples in the instructions, but some indexers use the date following the name and some use the date at the end of the line on the R side of the page. Any guidance would be appreciated.

    I think you would use the most recent date according to the field help:

    If multiple dates were given for the same transaction, index the most recent date.

    This form is most like Sample One and the indexer was correct to select the most recent date from the end of the transaction.
