The record is an application to reopen a grave for burial of an additional person. The death date is not recorded, nor the actual date of burial. The dates provided are the date of the application, the stamped date by the crime prevention police unit, the date acknowledging sworn statement of the applicant, and the proposed date of burial.

Shall I leave the burial date blank?

In the instructions we find the following:

    • The issue date may be recorded as the burial date on some records if a date is needed.
    • The issue date or burial order date may be indexed as the burial date.
    • If there are multiple burial dates recorded for an individual, index the latest burial date.

With those statements it would appear that we are being instructed to get as close as possible to the actual burial date as we possibly can. On your image #2 the proposed burial date is 20/02/2010. The application was approved so I would use the proposed date as the burial date.

    Hope that is helpful🙂
