Should I index?
Hi,
I would like to ask, should I index messages (?) next to the document?
For example, the current document that I am indexing has a page where the father's name was crossed out, indicating that the father's name is unknown, but next to it there is a... let us call it a message, that says that XY acknowledges the child mentioned in the document.
Here is the aforementioned document:
So my question, should I index the XY as the father or not?

If XY acknowledge that they are the child's father, then you should index that information. You can use any information found on the document - it doesn't have to be on the appropriate line. Also, this could be considered a situation for the rule of crossed out and replaced information.
