Wisconsin Divorce Records Oct 29 - Wife's Surname

The example given in the instructions shows using the surname in the current last name box (4). But the instructions, when I download a new batch, states to look at the wife's new name (18) for further information. Which name goes in the "spouses surname" box - line 4 or line 18?

    I am part of the special project and can access and index the Wisconsin records, but somehow I can't log on to the community.

    I have the same question as KathyBennettEdwards about the wife's new name. What goes in the wife's surname box--her current name followed by OR and the new name?
