Confusion in instructions regarding "registration date" as the signed date, but the field says "state file date" which is that date at the top that we used on the previous project. Which is correct to use for State file date fields?

I have the same question concerning "state file date".

It ask for the county of marriage, but usually it give the state where marriage took place do we just mark this blank or should we put in the state?
