Help with South African Death Notices

I have two images, both pertaining to the same person, and both are the same document. Someone else sort of asked this question, and was told to index them separately, but I'm not sure that this would apply here because both pages are one form, and the information would be very confusing if not combined. For instance, the name of the deceased is on the first page and when she died, but the actual date of the document is on the second page, with no other relevant information. What do I do here?

    Index 23 Aug 2005 as the record date along with the name and date of death, then mark Image 2 No, No Extractable Data.
