New Jersey birth indexes with parent initials

I'm indexing a batch of New Jersey birth indexes. Most records have the child's name. A few have the parent's initials instead of the child's name.

Should I mark the child's name as blank, enter the parent's initials or something else? I've tried adding the parent's name field for that record, but then that creates a field for all the records in the batch, which is not provided for the other records.

    According to the PI:

If the parents' initials are recorded rather than the child's given name, the first initial is indexed as the father's given name, the second initial is indexed as the mother's given name, and the "Surname" column is indexed for the surnames of the child, the father, and the mother.

Ex: If you have Smith as the surname and then A. & B. under Christian Name or Parents' Names, you will mark the child's given name blank, then the surname of the child and both parents will be Smith, with the father's name A. and the mother's name B. This is also the case for parents who have the same initial (like Smith, J. & J.).
