Washington Death Registers -- Am I interpreting these columns correctly?

Could someone take look at my batch to see if I'm interpreting the columns correctly? I've done a few batches, but the death place location on this particular batch is only 1-2 digits (vs 3 digits on the previous ones). That made me confuse the death location with death age.

    Yes, you are! That one is like the example for 4C in the What to Remember Section of the PIs. The header was City/County which must refer to the list on the left side of the Number Key Template.
