I am indexing UK—Principal Probate Registry, 1858–1957 [Part O][M9B9-XST].

The probate of Edward Jones dated June 5 states, "formerly... in the City of Manchester but late of Earlestown Newton-le-Willows in the County of Lancaster".

Do I index the former residence city in addition to the later one? Or just one of them?

I would index that the way you have using the last place of residence.
