Indexing Wisconsin Birth certificates.

How do I know if it is a certificate or a charge-out?

    maryellenstevensbarnes1: A Certificate is a recorded document such as a Birth Certificate. In general, some charge-out sheets are a record of an adoption or other information added by the state sometimes months after the child is born. These do not always show the birth date or place, parents' names or other information shown on a birth certificate. Charge-out forms are labeled "Charge-Out" at the top of the form.

    I haven't seen any charge-outs in this project. The Project Instruction examples show what a certificate is.

    Thank you! I haven't seen any either, but I have reviewed, and corrected several documents labeled as charge- out, so I just wanted to make sure I was doing it right.
