US, Indiana, Newton County—Marriages, 1957–2018 [M38H-DH5]

Do I include zip codes on the address of the applicant?

    No, you don't include zip codes. We only include USA if it is written on the form and the example shows us to index the country. The field help says to expand abbreviations, but, usually we only use United States for USA.

    So, on entry 2 you would index the residence as Lake Village, Newton, Indiana.

    Since USA is on the form for birthplace, I would index Illinois, United States

    I wouldn't index all those zeroes before the registration number. 2010-78 should be sufficient based on the field help instruction. (They were the 78th application of 2010).
