Adoption on Wisconsin Birth Records?

I'm working on the Wisconsin Birth records and I came across a certificate that shows a child with her given name (surname of biological father) and both of the biological parents are listed. Then there is a second name (different first and the surname of adoptive father) listed. The adoptive father is listed above the bio father. There is a note about the recording of the adoption with the county and a note about a new record in another book.

I'm not sure how to index this....

    In the Project Instructions under What To Remember About This Project, it says,

    "Many charge-out sheets are records of an adoption. If the charge-out sheet indicates an adoption or is a 69.15 or 69.33 form, do not index the child's full new name or the names of the birth parents. If the adoptive parents' names are listed, they should be indexed."

    Indexing the adoptive parents rather than the birth parents has been the general rule for years, but I know of one project that told us to index the birth parents instead. This is why we need to read the specific project instructions for each project we index. (The help center doesn't answer this question like it used to. It's very non-specific anymore.)
