1) In lower left of this page is an extra box to add a spouse. It may have been auto created. There was a research help posted suggesting a possible second spouse. There was no second spouse so I dismissed the suggestion. The extra box remains.
2) I have added a number of photos to various person's pages. How to get tagged portion to show as an icon when viewing a "portrait-formatted" tree? Possibly this is done by a webmaster who reviews attachments and tags?

I think both of your questions have been answered, but just so it's all together in one place....
1) What you perceive as an "extra" Add Spouse button is probably due to a parent-child relationship with just one parent. This situation can arise on purpose, such as for an illegitimate child of a parent married to someone other than the child's other parent, or it can be accidental or incorrect, for example when a family is incompletely merged.
2) The headshots or other pictures on a tree view are profile portraits. You can add or change a portrait by clicking on it (or the place where it goes) at the top left on any of the profile's pages (Details, Time Line, Sources, Collaborate, Memories). You'll have the option to upload a new picture, or to use a photo or other memory that you already uploaded previously.
