What should I say for the date on this muster roll?
United States—Enlisted and Officer Muster Rolls and Rosters, 1916–1939 [Part R][M3L6-R78]
I'm not sure what I should index as the date for this record. Would it be best just to leave it blank?
Also, if a muster roll says it was taken, say, "On the last day of April, 1918" would it be safe to index "April 30, 1918?"

This is a monthly muster roll, so I would index it as September 24, 1918. The PI (Project Instructions --> What To Remember About This Project) states:
When multiple dates are present on the image, please choose the military date based on the following hierarchy:
Enlistment date
Muster roll or monthly roster date
Any other applicable military date
If the image says "last day of", You will index it as April 30, 1918, October 31, 1917, etc.

So, I am unable to look at your reference pages to help you find another suitable date. All I can view is your batch, and I see a date stamped at the top left side of the page.
It looks like Sep 8, 1918 or 1919. You may be able to find the year on the reference pages prior to your batch or the pages after your batch. Most likely this is the date you need to use.
In the field help instructions it states this.

- Any other applicable military date
I do not see any enlistment dates on this page by each person. You should check previous pages (in your reference pages) to see if there is a muster roll date. If you see a muster roll date at the top of the previous page or several pages before, and can confirm that these pages are from the same record, use the date from the previous page. If there are more than 1 muster roll date, use the earliest date. So if it says Apr 30, 1918 through May 31,1918, you would index the Apr 30, 1918 date.
Last days of months, are exactly that. So you can check the month and see which day was the last and that is fine. So if there are 30 days in April, you would say Apr 30.
Hope this helps. I can't do much without seeing the reference pages prior to your current page. But that is something FS has to fix.

I did look at the previous images, and they were clearly a separate roster, although it was clearly 1918. The images after it also said 1918. I went ahead and submitted it as Sep 24, 1918, and hopefully that's good enough to help researchers find it. :D Thanks for your input! I appreciate it.

@Joel Kay Glassett Where was the Sep 24, 1918 date from? I completely missed it if it was located on your batch. Was it on a reference page?

It was at the top center of the page under "Monthly Muster Roll". Going by the PI, that's the military date. September 8 could be anything.

Like @erutherford said, it was on my batch, at the top center of the page. Looking at all the other dates on the page, it seems like that was the latest date available.
