Should I index the two "DO" under Henry at the bottom of the page?

    No you don't need to index duplicate names. "DO" is to show that the name is the same as above. So Henry Randall only needs to be indexed once even though it's on this page 3 times.


    Also the 5th name down is Louis F Ramponi
