Could someone look at this Ukraine batch I have indexed?

Is there someone who can look at this batch and tell me if I am doing it right? It is baptism records from L'viv, Ukraine in 1880. I have used the Latin List on FamilySearch to help me know what each column is. I am especially concerned about the parents' names.

    The parent names seems to be fine, the mothers have only a given name no surnames- you may want to Ctrl+B to leave the entry blank.
