If the page number, or batch number, has been crossed out & replaced with another number, is that considered to be an amended document? The directions say: "A document is considered amended if any field was crossed out"

"A document is considered amended if any field was crossed out, if information was replaced, or if the word "amended" was written somewhere on the document."

In your case the field was crossed out and the information was replaced. It does fit the definition for amended.

That is my understanding. The directions didn't specify only certain things that could be crossed out, so I always count that as an amendment.
