Death record has first and last name of widow.

The spouse is not mentioned. Do I take the surname of the widow and put it in the spouse surname field or leave it blank?

    It depends on the Project Instructions. If you click the purple question mark icon (Field Help) next to the spouse surname does it instruct to leave it blank? Sometimes it might read something like - 'don't use surnames of others for spouse'...

    If you include the batch code (after the Project name batch code xxx-xxx - combination of letters and numbers) here someone can take a look at it.
